2025 Descendant Enrollment Survey

Your answers are confidential. We use an independent contractor to administer the survey to protect your privacy, so Koniag will not know your specific responses. 

Each mailed survey packet should include a preaddressed and prepaid envelope and you can contact Koniag if you would like an additional one. Alternately, completed surveys can also be mailed to:

Koniag Shareholder Surveys, P.O. Box 5703, Hopkins, MN 55343-7063. 

Each Shareholder and registered Descendant was sent their individual PIN to their mailing address on record. Additionally, those who have supplied us with their email addresses were sent their PINs via email.

Please note that PINs are randomly generated and individual to each Shareholder and registered  Descendant, and as such can’t be reused.
